{School/district name} is excited to partner with Well Child next year to provide your students with annual physicals and eye exams at school during the school day.
You can go ahead and sign your child up for these services online at www.wellchild.com. A signed consent form and completed registration packet are required to provide these services to your child.
Well Child physicals are the same as an annual visit to a pediatrician or clinic. A licensed Family Nurse Practitioner examines your child here at school, and all results and any referral information are sent home to you in a sealed envelope.
Well Vision eye exams are given by a licensed optometrist who will prescribe glasses if your child needs them. Well Child will then deliver those glasses to your child at school.
Well Child partners with a number of insurance providers and will bill your insurance provider for these exams.
